- We accept cash, e-check, and credit card. An additional processing fee will apply for e-check and credit cards.
- Please verify with your insurance provider what services are covered if you are unsure, prior to your visit.
- Insurance Status:
- Most insurance forms are accepted and qualify for services with no out-of-pocket costs.
- Individuals without insurance or who are uninsured or underinsured may qualify for free or reduced-cost services through our funding partners.
- If we are unable to determine your insurance eligibility, your appointment may be rescheduled.
Immunizations
- Immunizations are available for children and adults with or without insurance. We are able to bill most insurance through our partnership with VaxCare. We also participate in the Vaccines For Children (VFC) and adult publicly funded vaccines (317) Program.
- Please submit all available vaccination records prior to your appointment so that we can determine the most appropriate vaccinations for you and/or your child. Although we are able to access Indiana's vaccination registry (CHIRP) for prior vaccination history, most adults and some children do not have a complete record available in CHIRP. There is no national registry.
Please note:
- If you are sick or not feeling well, call 317-537-0500 to reschedule.
- Masks are required.
- Please bring your driver's license (or other state-issued identification) and health insurance card with you.
- We vaccinate everyone regardless of insurance status.
Childhood vaccines
Most students on a regular vaccine schedule will need vaccines prior to entering Kindergarten, 6th grade and 12th grade. All students are recommended to be up to date on the latest COVID-19 vaccine recommendations, and to receive an annual flu vaccine. Please refer to the Required & Recommended School Immunizations schedule for Indiana's full requirements. COVID vaccines are always available at our clinic on an appointment basis. Refer to our COVID-19 Vaccines & Boosters webpage for eligibility information.
- We offer routine vaccinations for all ages according to the CDC's Child and Adolescent Immunization Schedule.
- We believe the optimum time to receive vaccinations is per the recommendations of the Advisory Committee on Immunization Practices. Most children receive the majority of vaccinations between the ages of birth to 18 months.
- Please refer to the Required and Recommended School Immunizations for full school requirements. Please be aware that some vaccines require more than one dose to complete the series. Typically, we recommend the following for those who are not behind on vaccines, in addition to the current COVID-19 vaccination recommendations:
- Kindergarten: DTaP, IPV, MMR, Varicella
- 6th Grade: Tdap, MCV4, HPV x 2
- 12th Grade: MCV4 (#2), MenB x 2
- Annually: Influenza (flu) vaccine in/around October
- Please make appointments well in advance of school required cut-off dates. We cannot guarantee appointment availability during peak times.
- It is normal for children to be afraid of "shots". Check the Do's and Don'ts for tips on how to speak with your child. Children and parents can watch a virtual reading of local Fishers author Daisy Ramani's children's book, Miss Violet Vaccine, where readers follow the main character, Zoey, who is anxious about getting a vaccination.
Adult Vaccines
- Adults unsure of what vaccines they may need can check the CDC's adult immunization schedule for current recommendations. Individuals with specific risks (i.e., immunocompromised, asplenia, heart or lung disease, diabetes) will have additional or alternate recommendations.
- In addition to current COVID-19 vaccination guidelines, we typically recommend:
- Hepatitis B: if never completed the series earlier
- Tdap: every 10 years (or for any penetrating wound, or any pregnancy)
- Zoster (SHINGRIX): at age 50
- Pneumococcal: at age 65
- HPV: Up to age 45 depending on shared clinical decision-making
- Influenza (flu) vaccine: Annually, in/around October
- We carry the Shingles vaccine (SHINGRIX) for insured patients or those willing to pay out of pocket. It is not available through the publicly funded vaccine program at this time.
Travel Vaccines
We offer routine vaccinations, but do not currently offer specialized vaccines for travel. For more information about requirements and recommendations for travel, please visit the Centers for Disease Control and Prevention and select your destination.
There are several vaccination clinics in central Indiana that offer specialized vaccines for travel. Appointments should be made well in advance to be assure vaccination coverage prior to travel.

Child Lead Testing
Lead testing is now available through our clinic. Lead is a toxic heavy metal that can be found in a child’s environment in items such as paint, pipes, old toys, ceramics, jewelry, cosmetics, and batteries. Children ages 6 and under are especially vulnerable when exposed to lead because it negatively affects brain and bone development. Testing is recommended for children under 2 as well as children ages 3-6 who have not been previously tested. Please book an appointment for your child’s lead test. Learn more about lead testing from the Indiana Department of Health and CDC.
Tuberculosis (TB) Testing
TB Skin Tests are available through our clinic for a $20 administration fee. We do not bill insurance for this service.
QuantiFERON-TB Gold blood testing for TB infection is available. Insurance billing will be an option along with a $20 administration fee, or you may self-pay for $40 ($20 administration fee and $20 for testing).
COVID-19 PCR Testing
COVID-19 PCR tests are available to be scheduled online for individuals ages five (5) and older. At present, COVID-19 tests are offered free of charge. Appointments are required. Testing is performed at your vehicle. Please pull into one of the parking spots labeled for COVID-19 testing, remain in your vehicle and call the number on the placard to speak with clinic staff (317-537-0500). Please bring a form of photo identification. Results are typically available within 3 days and communicated electronically via our patient portal. For more information or questions, call the FHD Clinic at 317-537-0500.
HIV, Hepatitis C and STI Testing
Free HIV testing is anticipated to be available in early 2023.
Sexually Transmitted Infection (STI) testing services are anticipated to be available in the 2023. In the meantime, we recommend checking these locations for immediate testing or treatment needs: https://www.testing.com/std-testing/indianapolis-in/.
