Maximum out of pocket:
$850/individual, $1,500/family (in-network)
These totals include the deductible. After the max is met, 100% of covered in-network charges are covered.

$29.05/employee only, $94.76/employee + child(ren), 104.42/employee + spouse, $173.31/family
HIGH DEDUCTIBLE/HEATH SAVINGS ACCOUNT INSURANCE PLAN

$1500/individual (applies to single coverage only), $3000/family
Coinsurance:
100% after deductible is met
Maximum out of pocket:
In-Network: $1500/individual, $3000/family
AND
Out-of Network: $3000/individual, $6000/family
Town Health Savings Account Contribution:
$500/employee only, $1000/family (approved for 2012 only)
Employee Health Savings Account Contribution:
Amount elected by employee; Yearly Max: $3050/single, $6150/family
Bi-weekly Payroll Deduction for High Deductible insurance Plan (Includes: health, dental and vision):
$20.57/employee only, $71.51/employee + child(ren), $78.83/employee + spouse, $130.77/ family
Dental
100% preventative, 80% basic, 50% major up to $2,000/year with $25 annual deductible. You may visit any dentist, there is no network.
Vision through Vision Service Plus (VSP)
$10 co-pay for exam, $25 co-pay for materials
